The 38-year-old Cairns, who came out of retirement to join the Outlaws, hit six fours off 38 balls as his side reached their 150 target with two balls left.
He put on 63 with Outlaws captain Chris Read (29no), with Mark Ealham hitting the winning runs off James Allenby.
HD Ackerman (55 off 45) top-scored for the Foxes in their 149-7.
The South African's innings, including six fours, was the Kolpak player's third consecutive half-century and helped Leicestershire to a strong position of 114-3 in the 14th over.
But the loss of three wickets in 11 balls disrupted their progress and they only added 35 to the total in the final six overs, with Samit Patel and Rob Ferley taking two wickets apiece.
Nottinghamshire appeared in trouble at the start of the chase after seeing Adam Voges and Patel fall in the opening two overs, but got back in the game with five fours from Will Jefferson.
Bilal Shafayat then compiled a patient 27 before he was bowled by spinner Claude Henderson, an innings that set the scene for Cairns and Read to up the tempo before Ealham saw Notts home.
